Guiding opinions on the application and development of big data in health care

People’s governments of all provinces, autonomous regions and municipalities directly under the Central Government, ministries and commissions and institutions directly under the State Council:
Health care big data is an important basic strategic resource of the country. The application and development of big data in health care will bring about profound changes in the health care model, which is conducive to stimulating the motivation and vitality of deepening the reform of the medical and health system, improving the efficiency and quality of health care services, expanding the supply of resources, and constantly meeting the multi-level and diversified health needs of the people, which is conducive to cultivating new formats and economic growth points. In order to implement the requirements of the Notice of the State Council on Printing and Distributing the Action Plan for Promoting Big Data Development (Guo Fa [2015] No.50), conform to the development trend of emerging information technology, standardize and promote the integration, sharing and open application of health care big data, with the consent of the State Council, we hereby put forward the following opinions.
I. Guiding ideology, basic principles and development goals
(1) Guiding ideology. Thoroughly implement the spirit of the 18th CPC National Congress and the Third, Fourth and Fifth Plenary Sessions of the 18th CPC Central Committee, firmly establish and earnestly implement the development concept of innovation, coordination, green, openness and sharing, give play to the decisive role of the market in resource allocation according to the decision-making arrangements of the CPC Central Committee and the State Council, give better play to the role of the government, strengthen top-level design, consolidate the grassroots foundation, improve the policy system and innovate the working mechanism with the aim of ensuring the health of all people. Vigorously promote the interconnection, integration and open sharing of government health care information system and public health care data, eliminate information islands, actively create a development environment to promote the safety, standardization and innovative application of health care big data, explore new service modes and cultivate new formats through "internet plus Health Care", and strive to build a medical and health cause that people are satisfied with, so as to provide strong support for building a healthy China, building a well-off society in an all-round way and realizing the Chinese dream of the great rejuvenation of the Chinese nation.
(2) Basic principles.
Adhere to people-oriented and innovation-driven. Incorporate the application and development of health care big data into the national big data strategic layout, promote joint collaborative innovation between government and Industry-University-Research, strengthen basic research and key technology research, highlight key areas and key links of health care, use big data to expand service channels, extend and enrich service content, and better meet people’s health care needs.
Adhere to standardized, orderly, safe and controllable. Establish and improve laws and regulations on the opening and protection of health care big data, strengthen the construction of standards and security systems, strengthen the responsibility of security management, properly handle the relationship between application development and security, enhance the support capacity of security technology, and effectively protect personal privacy and information security.
Adhere to openness, integration, joint construction and sharing. Encourage cooperation between the government and social forces, adhere to overall planning, combination of distance and demonstration, focus on revitalizing and integrating existing resources, promote the formation of a good situation of support from all parties, opening up according to law, facilitating the people and benefiting the people, and fully release the data dividend to stimulate mass entrepreneurship and innovation.
(3) development goals. By the end of 2017, the national and provincial population health information platforms and the national drug bidding and purchasing business application platform will be interconnected, and a cross-departmental health care data resource sharing pattern will basically be formed. By 2020, the national open application platform for medical and health information will be built to realize cross-departmental and cross-regional sharing with basic data resources such as population, legal persons and spatial geography, and the data fusion application in medical, pharmaceutical, medical insurance and health-related fields has achieved remarkable results; Coordinate the regional layout, build 100 regional clinical medical data demonstration centers based on existing resources, basically realize that urban and rural residents have standardized electronic health records and fully functional health cards, continuously improve the policies and regulations, safety protection and application standard system of health care big data, basically establish the development model of health care big data application adapted to the national conditions, initially form the health care big data industry system, vigorously develop new formats, and get more benefits for the people.
Second, key tasks and major projects
(1) Consolidate the application foundation of big data in health care.
1 accelerate the construction of a unified, authoritative and interconnected population health information platform. Implement the national health insurance informatization project, in accordance with the principle of safety first and privacy protection, fully rely on the national e-government extranet and the unified data sharing and exchange platform, expand and improve the existing facilities and resources, build a national, provincial, municipal and county-level population health information platform in an all-round way, and strengthen data collection, integrated sharing and business collaboration of application information systems such as public health, family planning, medical services, medical security, drug supply and comprehensive management. Innovate the management mode and promote the online handling of birth registration. Eliminate data barriers, unblock the data sharing channels among departments, regions and industries, explore the information exchange mechanism of socialized health care data, and promote the realization of health care data gathering on the platform, business matters handling on the platform, and government decision-making relying on the platform.
2. Promote the sharing and opening of health care big data resources. Encourage all kinds of medical and health institutions to promote the collection and storage of health care big data, strengthen application support and operation and maintenance technical support, and open up data resource sharing channels. Accelerate the construction and improvement of the basic database with residents’ electronic health records, electronic medical records and electronic prescriptions as the core. Establish a health care data sharing mechanism that is closely coordinated and centralized across departments such as health and family planning, Chinese medicine and education, science and technology, industry and information technology, public security, civil affairs, human resources and social security, environmental protection, agriculture, commerce, safety supervision, inspection and quarantine, food and drug supervision, sports, statistics, tourism, meteorology, insurance supervision, and disabled persons’ federations. Explore and promote the standardized access of data resources generated by wearable devices, smart health electronic products, and health care mobile applications to the population health information platform. Establish a national health care data resource directory system, formulate policies and norms for the open application of classified, graded and sub-domain health care big data, and steadily promote the opening of health care big data.
(2) Comprehensively deepen the application of big data in health care.
3. Promote the application of governance big data in the health care industry. Strengthen and deepen the evaluation and monitoring of medical and health system reform, strengthen the accurate statistics, prediction and evaluation of important data such as residents’ health status, and strongly support the planning and decision-making of healthy China construction. Comprehensive use of health care big data resources and information technology means, improve the hospital evaluation system, promote the deepening of public hospital reform, improve the modern hospital management system, and optimize the layout of medical and health resources. Strengthen the supervision of medical institutions, improve the monitoring mechanism for the income composition and changing trend of medical care, drugs and consumables, and cooperate with business information such as medical service price, medical insurance payment, drug bidding and purchasing, and drug use to promote the linkage reform of medical care, medical insurance and medicine.
4. Promote the application of big data in clinical and scientific research of health care. Relying on the existing resources, we will build a number of clinical medical data demonstration centers for cardiovascular and cerebrovascular diseases, tumors, geriatrics and pediatrics, integrate national medical big data resources such as genomics and protein genomics, and build a clinical decision support system. Promote the application of gene chip and sequencing technology in genetic disease diagnosis, early cancer diagnosis and disease prevention and detection, strengthen the safety management of population genetic information, and promote the development of precision medical technology. Focusing on the needs of clinical drug development for major diseases and common key technologies of drug industrialization, a mechanism for data fusion and sharing of drug side effects prediction and innovative drug research and development was established. Make full use of superior resources, optimize the layout of biomedical big data, systematically strengthen the integration and sharing of clinical and scientific research data resources, improve the efficiency of medical research and application, and promote the development of smart medical care relying on the National Clinical Medical Research Center and collaborative research network.
5. Promote the application of public health big data. Strengthen the construction of public health business information system, improve the national immunization program, network direct reporting, network first aid, occupational disease prevention and control, port public health risk early warning and decision-making and other information systems and the application functions of mobile emergency business platform, promote information sharing and business collaboration among medical institutions, public health institutions and port inspection and quarantine institutions, and comprehensively improve the ability of public health monitoring, evaluation and decision-making management. Integrate social network public information resources, improve the early warning mechanism of disease-sensitive information, timely grasp and dynamically analyze the international public health risks such as the disease occurrence trend of the whole population and the global epidemic information of infectious diseases, and improve the early warning and emergency response capabilities of public health emergencies. Integrate monitoring data of environmental sanitation, drinking water, health hazards, port medical vectors and nuclear biochemistry, and effectively evaluate social factors affecting health. Carry out biological monitoring of key infectious diseases, occupational diseases, imported infectious diseases at ports and medical vectors, integrate multi-source monitoring data of infectious diseases and occupational diseases, and establish a rapid identification network system for laboratory pathogen detection results to effectively prevent and control major diseases. We will promote the monitoring and evaluation of disease risk factors and the intelligent application of maternal and child health care, elderly health care and international travel health care, and popularize healthy lifestyles.
6. Cultivate new formats of big data application in health care. Strengthen key technologies such as storage and cleaning of massive data in health care, analysis and mining, and security and privacy protection. Actively encourage social forces to innovate and develop health care business, promote the deep integration of health care business and big data technology, accelerate the construction of health care big data industry chain, and continuously promote the coordinated development of health care and health care, pension, housekeeping and other service industries. Develop home health information services, standardize services such as online pharmacies and third-party distribution of medical logistics, and promote the development of industries such as Chinese medicine health preservation, healthy pension, health management, health consultation, health culture, physical fitness, healthy medical tourism, healthy environment and healthy diet.
7. Develop and promote digital health care intelligent equipment. Support the research and development of artificial intelligence technology related to health care, biological three-dimensional (3D) printing technology, medical robots, large medical equipment, health and rehabilitation AIDS, wearable devices and related micro-sensing devices. Accelerate the transformation of research and development achievements, improve the production and manufacturing level of digital medical equipment, Internet of Things equipment, intelligent health products, functional state detection of traditional Chinese medicine and health care instruments and equipment, and promote the upgrading of health care intelligent equipment industry.
(three) standardize and promote the "internet plus health care" service.
8. Develop smart and healthy medical services to facilitate the people and benefit the people. Give play to the leading role of high-quality medical resources, encourage social forces to participate, integrate online and offline resources, standardize the management of medical Internet of Things and health care application (APP), vigorously promote the application of Internet health consultation, online appointment and triage, mobile payment, inspection and test results inquiry, follow-up and tracking, and optimize the formation of standardized, shared and mutual trust diagnosis and treatment processes. Explore the Internet health care service model. Based on the contract service of family doctors, we will promote the application integration of residents’ health cards and social security cards, activate the application of residents’ electronic health records, and promote the integrated electronic health service covering the whole life cycle of prevention, treatment, rehabilitation and health management.
9. Establish telemedicine application system in an all-round way. Implement the Health China Cloud Service Plan, build an integrated platform for health care services, provide remote consultation, remote imaging, remote pathology and remote ECG diagnosis services, and improve the mutual recognition and sharing mechanism of inspection results. Promote data resource sharing and business collaboration between large hospitals and primary medical and health institutions, general practitioners and specialists, improve the graded diagnosis and treatment information system based on the Internet and big data technology, extend and enlarge the service capacity of medical and health institutions, and promote the "downward shift of focus and sinking of resources" in a targeted manner.
10. Promote the application of health care education and training. Support the establishment of a health care education and training cloud platform based on the National Open University of Health Care and supported by the massive open online course Alliance of Health Care Education in China, encourage the development of health care training materials in massive open online course, explore new Internet teaching modes and methods, organize high-quality teachers to promote the open sharing and online interaction of online medical education resources, distance training, remote surgery teaching, learning effectiveness evaluation and other applications, facilitate lifelong education of medical staff, and enhance the ability of primary health care services.
(4) Strengthen the construction of health care big data guarantee system.
11. Strengthen the construction of regulations and standards system. Formulate and improve laws and regulations on the application and development of big data in health care, strengthen the standardized management of residents’ health information services, clarify the right to use information, and effectively protect the legitimate rights and interests of all parties concerned. Improve the supporting service system for data open sharing, and establish a management system of "graded authorization, classified application, and consistent rights and responsibilities". Standardize the access standards in the field of big data application in health care, establish the integrity mechanism and exit mechanism of big data application, and strictly regulate the development, mining and application behavior of big data. Establish unified standards such as disease diagnosis codes, clinical medical terms, inspection specifications, drug application codes, information data interfaces and transmission protocols, and promote the standardization of health care big data products and service processes.
12. Promote the construction of a network trust system. Strengthen the digital identity management of health care, build a national unified identification of medical and health personnel and medical and health institutions trusted medical digital identity, electronic real-name authentication, data access control information system, actively promote the application of electronic signatures, and gradually establish a new health care management model with traceable service management, safe operation of diagnosis and treatment data and multi-party cooperation.
13. Strengthen the security of health care data. Accelerate the construction of health care data security system, establish a responsibility system for data security management, and formulate rules for marking, scientific classification, risk classification and security review. Formulate a population health information security plan, strengthen the national and regional population health information engineering technical capabilities, pay attention to content security and technical security, and ensure the independent, controllable, stable and safe national key information infrastructure and core systems. Carry out the reliability, controllability and security evaluation of big data platforms and service providers, as well as the security evaluation and risk assessment of applications, and establish software evaluation and security review systems such as security protection, system interconnection and sharing, and citizen privacy protection. Strengthen the safety monitoring and early warning of big data, establish a linkage mechanism of safety information notification and emergency response, establish and improve the safety working mechanism of "internet plus Health Care" service, improve the measures to resolve and respond to potential risks, strengthen the protection of important information involving national interests, public safety, patient privacy, business secrets, and strengthen the safety prevention of medical schools and scientific research institutions.
14. Strengthen the construction of health care information compound talent team. We will implement the national health care informatization talent development plan, strengthen the discipline construction of medical informatics and the cultivation of "digital doctors", focus on cultivating high-level and compound R&D talents and scientific research teams, and cultivate a group of internationally influential professionals, academic leaders and industry leaders. Innovate the form of continuing education for professionals, improve the multi-level and multi-type talent training system, promote the joint training of talents by the government, universities, research institutes, medical institutions and enterprises, and promote the construction of health care big data talents.
Third, strengthen organization and implementation
(1) Strengthen overall planning. Establish a working pattern of party Committee and government leadership, multi-party participation, resource sharing and coordinated promotion. The National Health and Family Planning Commission should comprehensively plan and strengthen implementation, and all relevant departments should closely cooperate and form a joint force to promote the implementation of key tasks. All regions should attach importance to the development of big data application in health care, earnestly do a good job in overall planning, infrastructure construction and safety supervision, and ensure that all tasks and measures are implemented. Promote the development of health care big data in integration of defense and civilian technologies, and promote the convergence, sharing and collaborative application of military and local health care data norms. Strengthen the guidance for the development of big data application in health care, strengthen the overall coordination of technology research and development, new format construction and application promotion, study and establish expert committees, and organize research and formulation of development strategies and related policies, regulations and standards.
(2) Grasp the key points and make breakthroughs. Starting from the areas urgently needed by the people, we will focus on promoting online appointment and triage, telemedicine, and sharing and mutual recognition of inspection results to facilitate the people and benefit the people. Accelerate the nationwide networking of basic medical insurance and settlement of medical treatment in different places. Support the development of medical smart devices and smart wearable devices, and strengthen research on key aspects such as difficult diseases. Select a number of areas and fields with good basic conditions, high work enthusiasm and guaranteed privacy and security to carry out health care big data application pilots, sum up experience, and make solid and orderly progress.
(3) Increase policy support. Study and formulate government support policies, and provide necessary support for the application and development of big data in health care from the aspects of finance, taxation, investment and innovation. Promote the use of government and social capital cooperation (PPP) model to encourage and guide social capital to participate in the basic engineering, application development and operation services of health care big data. Encourage the government to cooperate with enterprises, institutions and social institutions, and explore ways to integrate government applications and social applications in the field of health care big data through government procurement and social crowdsourcing. Give full play to the role of established investment funds, fully stimulate the enthusiasm of social capital and private capital to participate, encourage innovation in diversified investment mechanisms, improve risk prevention and supervision systems, and support the development of health care big data applications.
(4) Strengthen policy publicity and popularization. Strengthen the interpretation of health care big data application development policies, vigorously publicize the significance and application prospects of application development, actively respond to social concerns, and form a good social atmosphere. Actively guide medical and health institutions and social forces to participate in various forms of popular science activities, publicize and popularize the application knowledge of big data in health care, encourage the development of simple and easy digital medical tools, and continuously improve the people’s ability to master relevant applications and the public’s health literacy.
(5) Promoting international exchanges and cooperation. Orderly promote talent and technology exchanges and cooperation in the development of big data application in health care. Encourage relevant enterprises and scientific research units to introduce, digest, absorb and re-innovate international advanced technologies, and promote the simultaneous development of China’s independent technologies with the world. Increase the tracking, evaluation and transformation of international health care big data application standards, actively participate in the formulation of international standards, and enhance the right to speak in the formulation of relevant rules. Adhere to the principle of focusing on me, strengthening supervision and ensuring safety, steadily explore new modes of international cooperation in the application and development of health care big data, and continuously improve the application level, industrial core competitiveness and internationalization level of China’s health care big data.
